Waiver of Liability - Female Artists of Domination
During new member orientation, and at periodic intervals as required by the FAD Leadership team, you will be asked to sign the following waiver in order to partcipate in FAD events:
By my signature below, I agree that I have read, understand and will abide by the following:
- I acknowledge that I wish and desire to voluntarily participate in and/or observe BDSM activities and, in doing such, I am expressing something I consider to be a spiritual activity or sexual preference protected under my first amendment rights.
- I attest that if I am a reporter, a law enforcement officer or official or any form of investigator, I am not attending FAD functions in a capacity to gather information that may be used against FAD, its officers or members at a later date.
- I certify that I am 21 years of age and an adult capable of making sound decisions regarding my safety. I agree to play in a safe, sane and consensual manner should play be allowed at any function that I attend.
- I agree not to hold Female Artists of Domination (herein referred to as FAD), its officers or safety personnel, the organization in part or as a whole, or the owner of the space used liable for any injury or personal property damage that may be sustained while attending ANY function that FAD sponsors.
- I agree to abide by the rules and regulations set apart by the host or hostesses involved in providing the play space and the by-laws for my safety that have been established by the officers of FAD.
- I agree and understand that any Dungeon Monitor or designated FAD Officer will have total and complete access to ANY and ALL play areas. Any decision rendered by same regarding ANY scene that may be deemed to be unsafe will be binding, final and not questioned by me.
- I agree to remain quiet and orderly in the event that I am watching a scene take place, thereby not interrupting the play space nor the enjoyment of the people that are playing in that same play space.
- I agree that if I break any of the rules and regulations aforementioned in rule #3; that I may be asked to leave the function. I also understand that if the offense warrants my total expulsion from future events, that I will abide by the decision made by the officers in charge.
- I agree to clean up after any scene in which I am involved in any capacity, leaving the equipment in good repair for the next person or persons that may wish to use that equipment.
- I agree to co-mingle and interact with all of the members that attend any function in a respectable and dignified manner. I will not mistreat, abuse nor take liberties with any person attending, whether that person is submissive or Dominant, without explicit permission being granted by same.
- I understand that any cover charge collected to gain entry into the function will not be refunded for any reason. In the case of cover charges being collected before a function, cover charges will not be refunded if I choose not to attend after paying.
- I further understand that the cover charge is for this function only, that it does not, in anyway, apply towards my membership dues. I understand that membership dues are separate and are charged in addition to any cover charges paid to gain admittance to any FAD sponsored event.
- I understand that at no time will firearms; drugs; alcohol or weapons be allowed to be carried into a FAD function. I also understand that no prostitution or sexual bartering of any kind will be tolerated, and IS grounds for immediate expulsion if violated.
- I agree and understand that should a play scene involve the exchanging of bodily fluids, I agree to use protective barriers for my own protection, as well as the protection and safety of other attending members.
- I understand that this is a legal document and I attest that my identity is not falsified.
- I understand that this waiver of liability will be signed for each and every FAD sponsored event that I choose to attend.
- I attest that I have read and understand the House Collar Program sponsored by FAD.
